You voluntarily consent to receive the services and treatments outlined in your plan.

You understand that all medical aesthetic treatments carry potential risks, side effects, and variable results, which have been explained to you.

You confirm that all questions regarding your treatment have been answered to your satisfaction.

You consent to receive necessary communications related to your treatment via phone, text, or email.
