By completing payment in full, you acknowledge and agree to the following:

  • You have reviewed and accepted the treatment plan as discussed with your provider.
  • You voluntarily consent to receive the services and treatments outlined in your plan.
  • You understand that medical aesthetic treatments carry potential risks, side effects, and variable outcomes, all of which have been explained to you.
  • You confirm all your questions have been answered to your satisfaction.
  • You consent to receive all necessary communications related to your treatment, whether in person, by phone, email, or text.

Sculpt NYC LLC exclusively uses RHA® (Resilient Hyaluronic Acid) fillers from Revance, known for their advanced, natural-looking performance in dynamic facial areas.

By submitting full payment, you:

  • Confirm your informed consent and notification of side effects and risk involved to proceed with the treatment.
  • Acknowledge that you have not been coerced or misled.
  • Voluntarily assume any and all risks associated with the treatment.
  • Release Sculpt Medical Spas, its providers, employees, and affiliates from any and all liability related to the outcome of your treatment, to the fullest extent permitted by law.

This policy is effective immediately upon payment and applies to all services rendered at Sculpt NYC LLC, in compliance with New York State law.
